Boxer Health Questionnaire

We are grateful to receive all information relating to your Boxer’s health and/or temperament issues, even if these are not currently believed to be hereditary conditions.

If your boxer died within the last 3 years this data can be included.

We would also like to hear about healthy dogs and positive experiences with breeders. This will help us build up a better picture of the overall health of the breed.

Information about your dog (but not their KC. Registration number) will be published online, but you can opt-out if you prefer that your dog’s data is not used in this way. Your personal data will remain confidential, except if requested by an official body (such as The Kennel Club).

To help us evaluate the origins of these dogs and puppies we also ask you to tell us about your buying experience. These questions are optional so you can choose not to respond if you wish.

There is a tick box at the end of the form for those of you who do not wish any information to be published online but are happy for the information to be passed on to official groups.

By responding to this questionnaire you agree that your data can be shared with official bodies, such as The Kennel Club. However if you wish to change how your data is shared, or withdraw your personal data, you can email info@scottishboxerhealth.co.uk. All data is securely stored. Thank you.

9. Has your Boxer experienced any health issues or temperament issues? Please include all conditions, not just those you think they may have inherited.

10. Please state any direct medical effect & also the welfare effect of these conditions. It would be beneficial to include here details of any medical treatment (medication or operations), the costs and recovery times involved.  

Give as much factual detail as possible please. e.g the allergies cause raw painful skin on the paws and this affects her ability to walk and causes her pain.

8. Did you feel pressured at any point to buy the puppy/dog? 
9. Do you feel that both the parents & puppies living conditions were acceptable? 
10.  Did you feel you were buying a healthy puppy at the time or did you go against your better judgment and buy the puppy because you felt sorry for them, for example?  Please give details.
